Uncontrolled Keywords: Diabetes Mellitus merupakan penyakit metabolik yang bersifat kronik yang ditandai dengan meningkatnya kadar glukosa dalam darah. Kendali glukosa darah pasien DM dapat dicapai mulai dari perubahan pola hidup, obat anti hiperglikemi oral hingga insulin. Namun, masih ada berbagai macam alasan digunakan pasien untuk tidak rutin memakai insulin. Penelitian ini bertujuan untuk memahami alasan-alasan pasien tidak rutin menggunakan insulin di Rumah Sakit X Nusa Dua. Desain penelitian yang digunakan adalah kualitatif dengan pendekatan metode fenomenologi. Pemilihan informan dilakukan dengan purposive sampling dan Snowball dengan jumlah sebanyak tujuh orang. Pengumpulan data dilakukan dengan teknik wawancara mendalam, observasi dan di analisis secara Miles & Huberman menggunakan teori fenomenologi dan teori perilaku. Alasan-alasan pasien tidak rutin menggunakan insulin yaitu: tidak bersemangat, lupa, bosan, kurangnya dukungan keluarga. Informan dapat meningkatkan wawasan dan pengetahuan tentang pentingnya penggunaan terapi insulin pada pasien DM tipe II sebagai salah satu bentuk upaya mencegah komplikasi yang timbul
